All,

                Feedback from Ivana rightly pointed out that while the set of faults was specified, which fault would be returned in particular error cases was not specified.  This meant that two compliant implementations might choose to use two different faults for the same error case.

                Text has been updated mostly in that direction, with a few other minor changes.
