[bt-spec] New (final) model draft

I have put a revised draft of the model on the oasis website. This shows
change-marks, (with not-always accurate assignations of whole changes or
comments they are) relative to the previous version.

This text, with all its changes accepted is included in the 0.9.2.4 and
0.9.3 drafts. I suggest that comments be made against the review text,
rather than consider the model separately.
