For Option 1. Do you intend to re-use cmis:document for the annotation objects or do you intend to create an entirely new base type type 'cmis:annotation' that is a peer to cmis:document ?   I would suggest the latter.  And if yes, then you should have a separate section in the doc for this new object to define all of its properties, constraints, behaviors,  etc.
