I propose the following ranking:

IN:
- #45 Add See, See Also indexing elements

OUT:
- #45a Add sort order indexing elements
- #45b Add page range indexing elements
- #44 Keep indextermref (or redefine its function)

All of the requirements are important, but it feels to me that the
See/See Also is the most commonly requested and is a VERY common feature
of both online and printed help.
