Indexing proposals updated

I have uploaded to the TC archive slightly updated versions of the DITA 1.1 indexing proposals. A brief summary of changes:

• All 3 documents: fixed DTD typos. The indexing-d-index-base parameter entity was missing the  index-sort-as element (thanks, Don), and the class attributes were missing a trailing space (Thanks, Robert).
• Issue 45a: Add sort order indexing element: renamed index-sort-order to index-sort-as, as we discussed. There was some talk about confusion regarding the terminology. I also reworded the second sentence to make the meaning hopefully clearer: "This proposal addresses the ability to specify a sort phrase under which an index entry would be sorted".
• Issue 45b: Add page range indexing elements: there were objections that an ending page range marker should not generate an index entry. I have added clarification to that effect.
