Moves the <booktitle> element from a domain into bookmap - there is really
no reason to keep it in a domain. The name was changed from
<complexbooktitle> to simply <booktitle>. This zip also includes several
corrections to other modules, but nothing significant.

* Common modules shared with topic; 
* Domains modules; 
* A sample bookmap, slightly revised from the demo map in the current
toolkit.
