DITA Spec comments

Copying the DITA and dita-comment lists - there are a few minor bugs we've
noticed in further reviewing the spec.

1) The <anchor> element uses the generic ID definition, and shows that it
is not required. This should be updated with a specific definition for
<anchor>.

2) The examples in the foreign generalization topic show a <mathml> start
tag and end with a <mathcontent> tag - it should end with </mathml>

3) The two links for color information at w3.org is invalid:

4) In various locations the DITA filter attributes are listed, and rev is
specifically excluded as a filter attribute. It should not be included in
the description of @att in the attribute table here:
