Action: Provide correctly-named files for glossentry topic type

I have completed this action. The new and updated DTD and XSD files, and
updated master entity resolution catalog are in the SVN repository under the
doctypes/ tree.

I did the following:

1. Copied the original glossary* DTD and XSD files to glossentry*
2. Modified the original glossary* files to be nothing more than an
inclusion of the corresponding glossentry* file.
3. Updated all standard shells that used the glossary module to use
glossentry instead.
4. Updated the catalog-dita_template.xml file to provide public IDs and URNs
for the new glossentry* files.
