I *think* the significant piece of this output is the last half-dozen lines 
and I *believe* they indicate an incompatibility between or among the 
version of the FO specification you are generating and the version of 
PasiveTeX you are using.

The FO specification, just before it became final, was changed slightly 
with respect to the naming of some attributes and their values. In order to 
comply with the standard, you have to make sure all of your tools are up to 
date in this regard. It may be as simple as ensuring latest versions of 
style sheets, PassiveTex.
