I guess that's true. I suppose I'd write an XSLT to transform the TOC
into the TOC I wanted. Maybe that's just me :-)

It might. I'm not sure I've figured out how useful maketoc and
chunktoc are, actually.

Yes. I have an open action on myself to summarize them somewhere.
