The books I know don't do a pagebreak for the scenario above.

I think what you want is:
- do a pagebreak before every level 1 section
- do a pagebreak before every other section, unless it's the first in 
its hierarchy level

Regarding the stylesheet customization, some other kind soul has to help 
you on this.
