Take a look at any of those page masters fo/pagesetup.xsl, such as: <fo:simple-page-master master-name="blank-draft"> It sets a bunch of extra attributes on the <fo:region-body>. If you are using FOP, notice in that template that it doesn't output the background image if $fop.extensions isn't zero, because FOP doesn't support all the attributes for background images.
