Stylesheet questions should typically be sent to the docbook-apps list,
rather than docbook.  This list is more geared towards people discussing the
markup itself rather than the processing.

That said, I will quickly make a couple of suggestions on your problems:

1: The book title above the legal notice is generated by the titlepage
mechanism.  This is somewhat complicated at first, but very useful once you
get the hang of it.  It is documented at
http://www.sagehill.net/docbookxsl/TitlePagePrint.html

2: This should be the default behavior.  However, if I recall correctly,
this is a feature of FO that is not yet properly supported in FOP.

3: The templates header.content and footer.content control this.  They are
documented at http://www.sagehill.net/docbookxsl/PrintHeaders.html

4: See #3

5: This may be another FOP limitation if you're using FOP.  If print quality
is important to you, I strongly recommend paying the money for a commercial
FO formatter such as XEP.
