XSL Toolchain Round 2

Thanks to the suggestions of leaving out the FOP-specific patch I have  
managed to improve my situation somewhat.  Where I used to get:

[ERROR]  
file:/Users/jafoster/Documents/Academic/Projects/ 
Discipline%20of%20Design/DiscOfDesign.fo.fop:2:28750 master-reference  
'' for fo:page-sequence matches no simple-page-master or  
page-sequence-master

Now I get:

[ERROR]  
file:/Users/jafoster/Documents/Academic/Projects/ 
Discipline%20of%20Design/test.fo:2:47057 Flow 'xsl-region-body' does  
not map to the region-body in page-master 'blank'

I've tried both my document and a completely stripped down document  
with the same result.  To the best of my knowledge I'm not using a  
customization layer, nor anything outside of the FOP and DocBook  
distributions.

Hopefully someone out there can suggest another approach to making FOP  
work.  Thanks!
