Is there a DOCTYPE declaration in your doc??

<!DOCTYPE BOOK PUBLIC "-//OASIS//DTD DocBook V4.1//EN" >

(just an example -BOOK, of course).

The enviroment variable SGML_CATALOG should point to your docbook catalog.
