Empty indexes with profile-chunk.xsl

I'm experiencing some strange behavior moving to profile-chunk.xsl (from
chunk.xsl).  The created index html page is now empty. The html page for the
index is created (and participates in the toc) but is empty with exception
for the navigation and title elements. None of the indexterms content makes
it. (The indexterm blocks are not involved in 'userlevel' elements used to
hide specific paragraphs and notes)

Any suggestions on possible cause?
