I've resolved the error.

My apologies, I did not know that I needed to download a revised version 
of the htmlhelp-common.xsl file. As soon as that file was in place, 
everything worked extremely well!

I feel foolish--I should have double-checked to make sure I had all the 
right files. Thank you so much for the help.
