I don't see any indexterms in your document. I suppose that you are 
using some customization that puts all titles into index as indexterms. 
Problem is that HTML Help uses comma as a level separator in index term. 
I suppose that only solution is to modify this customization to replace 
comma by some other character that will look similar.
