Commercial formatters such as XEP and Antenna House have XSL-FO extensions
that deal with this. The problem is not that easy to fix with FOP,
unfortunately. However, you might be interested in Ken Holman's back of the
book index methodology. It takes some time to understand and set up, but it
could be worth a try:
