The problem with the header and footer rules not extending across the page
is a bug, described further in this message:

I didn't have much luck using $body.start.indent in FOP.  That's why the
default settings use $title.margin.left instead of $body.start.indent if
$fop.extensions is nonzero (i.e., FOP is being used).  If you are using
$body.start.indent, it should not be on the titlepage.  The template named
set.flow.properties in fo/pagesetup.xsl should exclude titlepage masters
from using that property.
