We had the similar problem and solved it by translating 
DocBook imageobjectco//area to HTML img/area
in XSL custom layer.
Please let me know if you need the code.
It isn't quite generic but it works.

You probably don't need several areasets --
several areas with distinct linkends would suffice.
The problem is that you can only refer to DocBook entities this way.
Which was OK for us, but you'd probably need ulink-style url attribute.
It's a pity it isn't in the DTD, yet you can customize it too.
