OK, I just checked in a change that (I think) causes an .sp macro
to be output instead of an empty line for that case. I did the
same for one or two other instances where I found that the
stylesheet was generating empty lines.

Please check with the latest snapshot and let me know if you find
any remaining instances of blank lines in your output.

At least in checking with the set of documents I've been testing
with, I've found that the only instances in which blank lines are
output is if they occur within verbatim environments, where it is
necessary and expected that they be preserved.
