I don't think the stylesheets promise to generate valid xhtml? 
  Bob, is that still true?

There was a stirring recently on a simpler docbook, perhaps
with valid XHTML as a possible output format, but nothing seems
to have taken.

I guess the complexity of docbook 4.x are such that it
will always be possible to write content that will not
generate valid XHTML.
