Hi Tom,
I can't duplicate this problem on Windows with the 1.69.1 stylesheets.
Using fop's -ps option to generate PostScript output, and then processing
that with Acrobat Distiller 6.0, I get the chapter headers showing at the
top of the resulting PDF pages (when double.sided = 0).

How are you converting to PDF on the Mac?  Are you using a stylesheet
customization?  If so, does it work with the stock stylesheet?
