Sorry I should have said that I am using FOP 0.20.5

I tried passing the image path as url() and plain. FOP docs say just use
<fo:external-graphic src="../images/logos/tutuka_300x116.png"/>

When I add the fo:external-graphic the entire header is blank. Comment it out 
and the text comes back in the other columns.
