I haven't tried this customization with recent docbook xsls, so it probably needs to be updated, but you can post-process the xsl-fo produced by (a customization of) the docbook xsls using Ken Holman's psmi stylesheets <http://www.cranesoftwrights.com/resources/psmi/> to landscape things as long as they aren't deep in the resulting fo tree.
