How to include sidebars in the table of contents...

I am automatically generating a ToC and want to include blocks marked with the
<sidebar> tag in the table of contents (note that I have my examples tagged as
a sidebar with a role of example) so that it appears like this:

1.  Introduction
1.1 Why...
1.2 Who...
    Example 1: title
    Example 2: title
1.3 ...
1.4 ...

Where/how do I override the template to include do this in the Toc? Is this a
case where it would be better to manually generate a ToC?
