DocBook 5 adds a cover element to handle covers and other external
content (like spines for books and splash screens in HTML).  It is a new
element, so the transforms may not handle it yet.  It will probably be
very heavily customized by organizations, since cover styles are usually
quite dependent on the organization producing the document.
