No, XSL-FO can't compose multiple pages onto one sheet of paper.

Probably the easiest solution to your problem is to create simple A5 output without "middle big graphics", and then use some PDF manipulation tool to compose final PDF from bits. If you use some PDF library like itext you can even script and fully automate whole process.
