Duplicate IDs in XSL-FO

When using the DocBook XSL-FO stylesheets, via Ant and Saxon 6, I'm 
getting a lot of duplicate ID errors. I.e. the same ID is being 
generated for multiple apparently unrelated items in the table of 
contents. This isn't for all items, but it's for enough to confuse Fop.

Any chance this is a DocBook XSL error, or is it more likely to be a 
Saxon bug?
