Hi Daniel,
It works for me.  I processed an article with indexterm elements and an 
empty index element at the end. I used xsltproc, the 1.72.0 fo stylesheets, 
and fop 0.20.5, and I get an index at the end.

I presume you have the empty index element if you are getting an index in 
the HTML output.
