Thanks for the detail.  If you are using the docbook5 stylesheets, then 
your document will work best when it is a DocBook 5 document, which means 
it must be in the DocBook 5 namespace.

Change:

<book>

to

<book xmlns="http://docbook.org/ns/docbook"; version="5.0">
