[docbook-tc] DocBook V4.2b0 "available"

We talked about doing a 4.2 release asap, so I've put together a first stab
at the release. It's available, though not publically linked to, at

  http://www.oasis-open.org/docbook/sgml/4.2b0/

and

  http://www.oasis-open.org/docbook/xml/4.2b0/

The 4.2 spec is

  http://www.oasis-open.org/docbook/docbook-4.2b0.html

We'll need to coordinate a release of the XML Character Entities with
this release because I've removed them from the 4.2 XML distribution.

Comments, etc. most welcome.
