Duplicated definition in callouts.rnc

Hi Norm,

there is duplicate line (lines 39 and 43) in callouts.rnc:

db.computeroutput.inlines |= db.co

Fortunatelly |= operator doesn't affect semantic when used twice so
there is no need for generating new schema release. But it should be
fixed in the next version.
