I think that would be the best option if you wanted to do this. For
one thing, the auto indexing code is about as hairy as XSLT gets.
Supporting more elements as indexterms is probably possible (might not
even be that hard, thinking about the code without looking at it) but
I'd like to avoid making that code any harder to grok.

But more importantly, good indexes are authored by hand. It's true
that in some documents most databases, filenames, etc., probably
appear in the glossary but it's not universally true that they all
should always appear there.
