I couldn't find one up to now. It can't be done with just stylesheet parameters in the stock DocBook stylesheets. It requires a stylesheet customization file containing a custom XSL template. It lets you set static links and images in headers and footers with parameters in a configuration file. But it assumes you are writing individual web pages, not books.
