It's already there, and I use it. Read up on the zone attribute of indexterm, http://docbook.org/tdg/en/html/indexterm.html. All that's required is an ID on the element that you want the indexterm to relate to. BTW, I'd also like to be able to assign types to an indexterm. Indexes are content-based, and I cannot think of a way that would allow you to create an index without inserting index term markup in the document. <snip/>
