It's already there, and I use it.

Read up on the zone attribute of indexterm,
http://docbook.org/tdg/en/html/indexterm.html.

All that's required is an ID on the element that you want the indexterm to
relate to.

BTW, I'd also like to be able to assign types to an indexterm. Types in the
plural; make it NMTOKENS.

Tobias,
I am not sure that I understand what you mean by separating the document 
from its indexes. Indexes are content-based, and I cannot think of a way 
that would allow you to create an index without inserting index term 
markup in the document.
