Forgive me if it's not in direct combination with DocBook, but the e-GMS
framework proposed by UK e-Gov builts upon DCQ:

The site has lots of documentation and an XSD schema for download.
I'm using it for documentation purposes and it's quite powerful.

As far as I've heard, something like DC will be built into the next
version of DocBook (based on RelaxNG). Until then, we'd have to go with
a stand-off metadatabase ...
