It looks like you are processing your fo output with FOP, but you aren't
setting the fop.extensions parameter to 1.  This is necessary to get the
left indents working properly with FOP.

Regarding the index columns, the default number of columns is 2, so you
should not have to set a parameter to get that.  I'm not sure why FOP
doesn't give you two columns.
