There are source files in CVS for the HTML forms extension (RELAX
NG compact syntax ones -- htmlform.rnc for the actual forms
content, along with a dbforms.rnc driver), but there appears to be
a build error that is causing the full/flattened .rnc/.rng/.dtd
files to not be built from that source correctly.

I'm sure it'll be resolved for the next DocBook 5 beta release.
In the mean time, I guess the workaround is to use the DocBook +
HTML forms grammar in the Lillet release, or to take the source
dbforms.rnc from CVS and build a .rng file from it manually.
