Using procedures in generates XPath errors in FO generation

Hello All,

 I'm writting an article using DocBook 5 and DocBook XSL 1.69.1, with 
xsltproc.  If I include a <procedure> within my document the FO output 
fails:

/usr/bin/xsltproc  /home/benr//DocBook/docbook-xsl-1.69.1/fo/docbook.xsl 
LDAP-article.xml > LDAP-article.fo
Making portrait pages on USletter paper (8.5inx11in)
XPath error : Invalid context position
xmlXPathCompiledEval: evaluation failed


If I remove the procedure completely from my document it renders 
perfectly.  As a test I removed my procedure and inserted the example 
from the element reference and it still fails, so it doesn't appear to 
be the way I'm using it.

Is anyone familiar with this problem or have a solution?  HTML output 
works fine, but I need to produce PDF's in the  next couple days.
