This may not help you, since DocBook v5 is still under development, but it has a new annotation element that would be appropriate for this purpose. An annotation element can contain an info element to track author, and a nice mix of other elements for expressing comments. And its 'annotates' attribute is meant to point to the element it is annotating, which is not necessarily the element containing it. So you could annotate another annotation to string together a sequence of comments. There is some experimental support in the html stylesheet for the annotation element. See the html/annotations.xsl stylesheet file. But your application would require a customization, probably using roles on the annotations so they would be handled the way you expect them to be. If you need to use DocBook4, then the remark element is your best bet. It accepts author and revhistory as part of its content mix. You can even nest remarks inside remarks.
