It is difficult to draw line there. You can do content reuse even on
inline elements, so you can end up with having all elements in the start
pattern. But this would make schema less useful for typical usage, when
there is good that list of root elements is constrained.

DocBook TC made compromise there including only "component" elements in
the start pattern. In RELAX NG it is easy to customize schema, so you
can extend this list if necessary. But if you think that DocBook schema
should contain more elements in the start pattern please feel free to
raise this request at

in next release of DocBook.
