Bob, Thanks for your reply. I am not getting refentries in my TOC in FO (PDF) or in my HTML output. I am running Fedora 20 which uses xsl-stylesheets-1.78.1 If you look the output I am producing at http://documentation-us.gazillabyte.com/book_sync.pdf There are about 40 man pages embedded in the document. None are being added to the TOC. In most cases the refentries are a running part of book->part->chapter. They are all also included with xinclude (not sure if that is part of the problem).
