Just in case the format isn't completely apparent:

4. (the number refers to the number of the issue even if it is only a typographical error)

Sylvia Candelaria de Ram, (the name refers to the person who raised this, usually Sylvia because she took on the proofreading chores which I guarantee you are absolutely necessary and for which we thank her greatly)

Section 4.2.2 (and that is followed by the Section Number)

Artifact, (Name of the Item)

minor changes in <xs:documentation> (a brief description, which is mostly the same as this one)

From: "... trinkets, etc., artifacts are often used..." (what is being changed)

To: "... trinkets, etc., are artifacts often used...", (the change)

Giving us:
