regarding the start-value. Another source of confusion I think.

Can you give some XML source using the start-value? A sample please; a kingdom for a sample :-)

This makes no sense to me. Of course the numbering continues since those items are part of the same list.
There is no need for the continue-numbering attribute at all for numbered-paragraphs, IMHO.

If we want
1
2
1
we can simply use start-value="1" for the last paragraph, right?
