If I understood correctly then the start-value is only considered when continue-numbering is "false"?

Why? It restarts the numbering at the given numbered paragraph, using the specified value.

I don't see any problem with:
<numbered-paragraph style-name="L1" list-id="1"><p>P1</p></numbered-paragaph>
<numbered-paragraph style-name="L1" list-id="1"><p>P2</p></numbered-paragaph>
<numbered-paragraph style-name="L1" list-id="1" start-value="1"><p>P3</p></numbered-paragaph>
<numbered-paragraph style-name="L1" list-id="1"><p>P4</p></numbered-paragaph>

1 P1
2 P2
1 P3
2 P4

(Isn't this exactly how it would work with list-item?)
