- the bookmark-ref / reference-ref elements identify their
  bookmark(-start) / reference-mark(-start) by its text:name attribute
- the sequence-ref identifies the corresponding text:sequence field by 
  the field's text:ref-name attribute (because the field uses text:name 
  for other purposes)
- the note-ref identifies the corresponding text:note by the note's 
  text:id attribute

[incidentally, why does text:note-ref have the attribute text:note-class?
 the note-class attribute is already at the text:note;
 this duplication would only make sense if the text:id namespace for 
 text:note elements would be defined per note-class, which i would find 
 surprising. will probably file an issue for this.]
