I have removed the "In all other cases" paragraph.

I am not sure what is being requested for the list of element values. We already end each description with:

The form:list-source attribute defines that evaluation. 19.287

What is unclear about that? 

We had this discussion and to avoid simply bulking up the standard with several thousand (see blah) have used the convention of simply listing the cross reference.
