SCA Assembly version 1.2 WD04 uploaded

I have uploaded both doc [1] and PDF [2] versions of v1.2 WD04.
This is based off Bryan's WD03_clean version [1]. I accepted all the 
changes in WD03_clean before making my changes.

This brings v1.2 in sync with v1.1 WD074 and pulls in the resolutions of 
all the 1.2 issues in JIRA that have a status of 'resolved'. I have also 
labeled 1.2 conformance items and included them in the appendix tables.

I'll update WD04 to pull in any changes that Mike makes between 
v1.1WD074 and v1.1CSD08. Sending this in advance so folks can review it.
